    I AM NOT in the WVASFAA database - Select this if you are new to WVASFAA.
  • Revise Your Sponsorship
    If you are returning to revise your form , use the "Revise Existing Form " section below. Enter your Username and Password and make any necessary changes. Your previously entered registration information will be deleted and replaced with the new.
  • Cancel Form
    Cancelling a Form? Select "Cancel Form " and enter your Username and Password. Your submission will be deleted from the records and cannot be recovered.
